Teens/young adults: well care, sports physicals, focus on developing healthy habits for life, a trusted adult to reach out to with their concerns.
Adults: routine physicals and well woman care, preventative care, weight management with individualized nutrition and exercise plans, screening for and managing chronic disease
Older adults: maintaining health and quality of life, managing chronic diseases, coordinating care with specialists as needed, house calls available.
